Sunbury Christian Academy has joined the PIAA in girl’s and boy’s varsity basketball. Attached is a medical evaluation that is required by all varsity athletes participating in PIAA sports. Most of the evaluation is to be completed by the parents and athletes, but SECTION 6: PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION AND CERTIFICATION OF AUTHORIZED MEDICAL EXAMINER must be completed and signed by a licensed physician.
Please make sure your child has a physical examination before the start of basketball pre-season practice. These forms must be turned into the school office upon completion in order for your child to eligible to play in PIAA games.
If your child requires medical treatment from a licensed physician of medicine or osteopathic medicine after the completion of SECTIONS 1 through SECTION 6, then SECTION 8: RE-CERTIFICATION BY LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE must be completed, signed off by a licensed physician and submitted to the school office before the athlete is allowed to return to PIAA sanctioned events. If your child is participating in more than one PIAA sport, then SECTION 7: RE-CERTIFICATION BY PARENT/GUARDIAN must be completed and returned to the school office.
SCA will continue to be a member of and compete in the Allegheny Christian Athletic Association (ACAA)
